PT-141 (bremelanotide) is a cyclic heptapeptide melanocortin receptor agonist that acts centrally, in the hypothalamus and limbic system, rather than peripherally on vascular tissue. That mechanism changes how it is dosed. There is no titration schedule to climb and no daily cadence to maintain, because the compound is used before anticipated activity and cleared within hours. The two decisions that actually determine whether a protocol works are the size of the first dose and how far in advance you inject it.

Standard PT-141 Dose Range

The working range is 1 to 2 mg per use, subcutaneous. The FDA-approved dose, marketed as Vyleesi for hypoactive sexual desire disorder in premenopausal women, is 1.75 mg delivered by autoinjector. Phase 2 dose-finding work tested 0.75 mg, 1.25 mg, and 1.75 mg, and the response curve flattens well before the top of that range while the side effect curve does not. That asymmetry is the entire argument for starting low.

  • Conservative first dose: 0.5 mg, appropriate if you are small framed, sensitive to nausea, or testing tolerance for the first time
  • Standard starting dose: 1 mg, effective for a large share of users and meaningfully easier on the stomach than 1.75 mg

Start at 1 mg, judge the effect at 2 hours, and only move up if the response was genuinely insufficient rather than merely later than expected.

Concentration Math: What to Draw on the Syringe

PT-141 is most often supplied as a 10 mg lyophilized vial, with 5 mg vials also common. Unlike nightly peptides where a 1 mg/mL mix keeps the arithmetic clean, PT-141 doses are large enough that 1 mg/mL forces a 100 unit draw for a single 1 mg dose, which will not fit in a 0.3 mL or 0.5 mL insulin syringe. Concentrating the vial more heavily is the correct move here.

  • 5 mg vial + 2.5 mL BAC water = 2 mg/mL: 0.5 mg = 25 units, 1 mg = 50 units
  • 5 mg vial + 2 mL BAC water = 2.5 mg/mL: 1 mg = 40 units, 1.75 mg = 70 units

If you plan to run 1.75 mg, the 2.5 mg/mL mix (10 mg vial with 4 mL) is the most practical: 70 units fits a 1 mL or 0.5 mL syringe with room to spare and the number is easy to hit accurately. If you plan to stay at 1 mg, the 2 mg/mL mix at 50 units is the cleanest. The 4 mg/mL mix minimizes injection volume and stings less, but a one unit misread costs you 40 mcg instead of 20 mcg, so it suits people who already dose confidently.

Vial economics matter more with PT-141 than with daily peptides, because a 10 mg vial holds 10 doses at 1 mg and only 5 doses at 2 mg, while the 28-day preservative window runs regardless of how many doses you actually take. If you use the compound twice a month, a 5 mg vial reconstituted in small volume, or splitting a 10 mg vial across two sterile vials, wastes less product than opening a full 10 mg vial you cannot finish.

Timing: The 45 to 60 Minute Window

PT-141 has a plasma half-life of roughly 2.7 hours. Onset of subjective effect typically begins 45 to 60 minutes after a subcutaneous injection, peaks around 2 hours, and the useful window extends 6 to 12 hours in most people. Injecting immediately before activity is the single most common dosing mistake, because the compound has no meaningful acute effect in the first half hour and users conclude it did not work.

  1. Inject 45 to 60 minutes before anticipated activity. If this is your first use, give it 90 minutes so you are not judging the dose too early.
  2. Swab the vial stopper with 70 percent isopropyl alcohol and let it air dry for 10 seconds.
  3. Draw the dose with a fresh 29-31 gauge U-100 insulin syringe, then tap out air bubbles before expelling.
  4. Pinch a fold of abdominal skin at least two inches from the navel and inject at 45 to 90 degrees.
  5. Stay upright or lie down through the first 60 to 90 minutes, whichever settles nausea better for you, and keep water available.
  6. Do not redose because the effect feels slow. Peak is at roughly 2 hours, not 30 minutes.

A light meal beforehand is fine and often reduces nausea, which is the opposite of the fasted requirement for growth hormone secretagogues. There is no insulin or somatostatin interaction to work around here, because PT-141 acts on melanocortin receptors rather than the pituitary GH axis. Alcohol is the more relevant variable: it blunts the subjective effect and compounds the blood pressure response, so heavy drinking around a dose is worth avoiding.

Dose Frequency and Monthly Limits

The frequency ceiling on PT-141 is stricter than on most research peptides, and it comes from the approved label rather than from convention. Bremelanotide is limited to one dose per 24 hours and no more than eight doses per calendar month. The 24-hour rule exists because the blood pressure response and the elimination curve overlap when doses stack, and the monthly cap reflects the fact that the phase 3 program never studied heavier use.

  • No loading phase, no daily maintenance dose, and no benefit to scheduled dosing between uses

PT-141 raises blood pressure transiently, on the order of 6 mmHg systolic and 3 mmHg diastolic on average, peaking 2 to 4 hours after injection and resolving within about 12 hours. It is contraindicated in uncontrolled hypertension and known cardiovascular disease. If you take antihypertensive medication, this is a conversation with a physician, not a dose you titrate on your own.

Side Effects and Interactions

Nausea is the dominant side effect, reported by roughly 40 percent of participants at 1.75 mg, usually in the first 30 to 90 minutes and usually mild to moderate. It is strongly dose dependent, which is why 1 mg is the sensible entry point. Flushing, headache, and injection site reactions follow in frequency. Vomiting occurs in a small minority and is far more common at the top of the dose range than at 1 mg.

Two interactions deserve attention. PT-141 slows gastric emptying, so oral medications taken in the same window may be absorbed more slowly or less completely, which matters most for drugs where timing is critical. Separately, bremelanotide substantially reduces exposure to oral naltrexone, so anyone using naltrexone for alcohol or opioid use disorder should not combine the two. Neither interaction is dose dependent in a way you can manage by lowering the dose.

Because PT-141 is a non-selective melanocortin agonist with activity at MC1R, repeated frequent dosing can produce darkening of existing moles and freckles or patchy hyperpigmentation, particularly on the face and gums. At intermittent as-needed use within the eight-doses-per-month ceiling this is uncommon. It becomes a real risk when users push toward daily dosing, which is one more reason the frequency limit is not arbitrary.

PT-141 vs PDE5 Inhibitors

Sildenafil and tadalafil are peripheral vasodilators: they inhibit PDE5, increase blood flow to genital tissue, and require pre-existing arousal to produce an effect. PT-141 works upstream of that, activating MC3R and MC4R receptors in the hypothalamus and increasing dopaminergic signaling associated with sexual motivation. The practical consequence is that PT-141 addresses desire rather than mechanics, works in both men and women, and has documented effect in men who did not respond adequately to PDE5 inhibitors.

The two classes are not interchangeable and are not additive in a simple way. Combining them stacks two blood pressure effects in opposite directions, PDE5 inhibitors lower it while PT-141 raises it transiently, which is exactly the kind of unpredictable interaction that makes cardiovascular screening non-negotiable. PT-141 vs Melanotan 2

PT-141 is the deliberate development product of Melanotan 2 (MT-II), an alpha-MSH analog originally pursued for tanning, after trials found pronounced sexual arousal as an unintended effect. PT-141 is the des-amino metabolite, engineered toward the MC3R and MC4R sexual function pathway and away from the MC1R pigmentation pathway. Melanotan 2 retains far stronger MC1R activity, which is why it tans reliably and why its side effect profile includes the pigmentation issues that PT-141 largely avoids at normal frequency. If sexual function is the goal, PT-141 is the targeted compound and Melanotan 2 is the blunt precursor.

Reconstituted PT-141 should be refrigerated at 2 to 8 degrees Celsius and used within 28 days, protected from light. The awkward part of an as-needed compound is that the clock starts at reconstitution, not at first meaningful use, so a vial mixed for one weekend expires with most of its contents intact. Lyophilized powder is considerably more stable and tolerates room temperature in transit, so leave vials unmixed until you expect to use them within the month.

Inspect the solution before every draw. It should be clear and colorless. Cloudiness, visible particulates, or discoloration mean the vial goes in the sharps container.
